The 29-year-old left Burnley during last January's transfer window to sign for the Blues, making his debut against Leeds in the third round of the FA Cup.
He opened his goal account the following month with a penalty in the 2-1 win over Southampton.
Blake has reportedly accepted an offer which will commit him to Elland Road until 2008.
